This drug can neutralize three BA.4/BA.5 subvariants, which should be monitored
A group of experts from Peking University, National Institutes for Food and Drug Control (NIFDC), Beijing Dital Hospital, Capital Medical University have discovered that the drug Bebtelovimab can detect and neutralize multiple BA.4 and BA.5 subvariants with R346 mutations (R346I, R346T, R346S). Their studies are published in preprint form by bioRxiv, an open access reposity for research hosted by Cold Spring Harbor Laboratory.
Sunney Xie Xiaoliang of Peking University revealed that the experts have done studies on the potency of approved neutralizing antibody drugs including 11 monoclonal antibodies and 4 cocktails such as Bebtelovimab, Evusheld, REGEN-COV and Sotrovimab. Only the potency of Bebtelovimab is guaranted. Bebtelovimab is the "only surviving BAb drug" approved by the U.S. Food and Drug Administration (FDA).

Bebtelovimab, the monoclonal antibody treatment from Eli Lilly & Company (above), is granted Emergency Use Authorization (EUA) by FDA in February 2022 to fight against the Omicron variant.
The experts found that Evusheld, made up of Tixagevimab and Cilganvimab, has completely lost its efficacy as BA.4/BA.5 with R346I, R346T and R346S completely escapes the detection by Cilgavimab. These R346 mutations also reduce the neutralizing activity of REGEN-COV (Casirivimab and Imdevimab) due to the decreased reactivity of Imdevimab against R346-mutated sublineages.
So is the potency of Sotrovimab further reduced by these R346 sublineages.
R346 is an important immunogenic residue and R346 mutations are found to be able to escape a large group of neutralizing antibodies.

Sinovac vaccine not effective against R346 sublineages
The experts also discover that individuals who received triple doses of Sinovac covi vaccine CoronaVac without infection showed 1.5 to 1.7-fold decrease in 50% neutralization titers (NT50) against BA.4/BA.5 with R346I (BA.5.9), R346T (BA.4.6 and BF.7) and R346S (BA.4.7).

After breakthrough infection comes subvariant evasion
The experts declare BA.5.9, BA.4.6 and BA.4.7 could significantly evade neutralization by plasma samples from BA.5 breakthrough infection. They measure the neutralizing titers of plasma samples against these SARS-CoV-2(covi) BA.4/BA.5 subvariants, which are obtained from vaccinated individuals without covi infection or with BA.1/BA.2/BA.5 breakthrough infection.
BA.5.9, BA.4.6 and BA.4.7 on the spoke glycoprotein have been identified in various countries including CCP China and the U.S. BA.4.6 harbours R346T, BA.4.7 harbours R346S while BA.5.9 harbours R346I. These subvariants exhibit growth advantages compared to BA.5 and BA.5.

Authorized use areas of Bebtelovimab
According to Eli Lilly, Bebtelovimab is authorized for use under EUA for the treatment of mild-to-moderate covid on adults and pediatric patients (12 years of age and older weighing at least 40 kg) including patients with positive covi testing results, who are at high risk of hospitalization or death and for whom alternate covid treatment options approved or authorized by FDA are not accessible or clinically inappropriate.

Adverse events from monoclonal antibody treatments
According to Eli Lilly, serious hypersensitivity reactions, including anaphylaxis, have been observed with administration of other covi monoclonal antibodies and could occur with administration of Bebtelovimab. If signs and symptoms of a clinically significant hypersensitivity reaction or anaphylaxis occur, immediately discontinue administration, and initiate appropriate medications and/or supportive care.
Infusion-related reactions, which may occur up to 24 hours after the injection, have been observed in clinical trials of Bebtelovimab when administered with other monoclonal antibodies and may occur with use of Bebtelovimab alone. These reactions may be severe or life threatening. Signs and symptoms of infusion-related reactions may include fever, difficulty breathing, reduced oxygen saturation, chills, fatigue, arrhythmia (e.g. atrial fibrillation, sinus tachycardia, bradycardia), chest pain or discomfort, weakness, altered mental status, nausea, headache, bronchospasm, hypotension, hypertension, angioedema, throat irritation, rash including urticaria, pruritus, myalgia, vasovagal reactions (e.g. pre-syncope, syncope), dizziness, and diaphoresis.
Hypersensitivity reactions occurring more than 24 hours after the injection have also been reported with the use of covi monoclonal antibodies under EUA.

Clinical worsening after monoclonal antibody administration
According to Eli Lilly, clinical worsening of covid patients after administration of covi monoclonal antibody treatment has been reported and may include signs or symptoms of fever, hypoxia or increased respiratory difficulty, arrhythmia (e.g., atrial fibrillation, sinus tachycardia, bradycardia), fatigue, and altered mental status. Some of these events required hospitalization. It is not known if these events were related to covi monoclonal antibody use or were due to progression of the disease.

Adverse reactions after receiving Bebtelovimab reported
Adverse reactions observed in those who have received Bebtelovimab alone or in combination with Bamlanivimab and Etesevimab, at the authorized dose or higher are infusion-related reactions (n=2, 0.3%), pruritus (n=2, 0.3%) and rash (n=5, 0.8%). The most common treatment-emergent adverse events observed in subjects treated with Bebtelovimab, alone or in combination with Bamlanivimab and Etesevimab, at the authorized dose or higher included nausea (0.8%) and vomiting (0.7%).
Therefore, not only vaccine injuries and vaccine adverse reactions need to be monitored, adverse events and reactions from monoclonal antibody administration need to be monitored as well. Drugs used in curing covid patients or preventing covid have their own limitations.
Bebtelovimab (LY-CoV1404; LY3853113) is a neutralizing IgG1 monoclonal antibody (mAb) directed against the spike protein of SARS-CoV-2 that maintains binding and neutralizing activity across currently known and reported variants of concern, including Omicron and BA.2. It is being studied for the treatment of mild-to-moderate covid both as a monotherapy and together with other mAbs. Lilly has licensed and developed Bebtelovimab after it was discovered by AbCellera and the scientists at the National Institute of Allergy and Infectious Diseases (NIAID) Vaccine Research Center.

Navarro's new book reveals why Republicans lost the White House

Former White House trade adviser Peter Navarro said that poor personnel appointments are doomed to failure of the Trump administration.
"I call it the Achilles' Heel of my boss. I love this boss despite all these bad arrangements. He's the best president we've ever had," Navarro said in a statement. He said in a recent interview with the English-language New Tang Dynasty Television (NTD) program "Capitol Report".
He also said that Trump's 2020 presidential campaign must not be repeated in a possible 2024 presidential bid.
Navarro is sharing content from his new book, Taking Back Trump's America: Why We Lost the White House and How We'll Win. The book will be published on 20 September 2022.
In his book, Navarro wrote about how Trump, soon after becoming elected, began surrounding himself with “globalists, Never-Trump Republicans, wild-eyed [House] Freedom Caucus nut jobs,” and “Wall Street transactionalists,” who would “come to plant so many poisonous Bad Policy Trees” during his administration.
Navarro said he was one of three senior advisers who stayed with Trump during the 2016 campaign until the handover. He was "relentless" when he commented on Trump's mistakes in personnel choices.
In the interview, Navarro cited Trump's first secretary of state, Rex Tillerson and Secretary of Defense Jim Mattis, as examples of Trump's "stupid choices," This will be part of what he calls a "clown cabinet".
According to his book, Tillerson and Mattis had opposed Trump's efforts to renegotiate a trade deal with South Korea, arguing that doing so could undermine "delicate military calculations" or "diplomatic alliances" between the two countries.
Despite their opposition, Trump and then-South Korean President Moon Jae-in signed a revised free trade agreement in September 2018 that was welcomed by U.S. automakers and meat exporters.
Trump has said the old deal, approved by Congress in 2011 and taking effect the following year, was a "terrible deal" by Hillary Clinton, who, as secretary of state in the Obama administration, pushed for the final version of the trade pact.
The U.S. trade deficit with South Korea increased from $16.7 billion in 2012 to $27.7 billion in 2016, according to the U.S. Trade Office.
China policy
One of the "people who have never met Trump" that Navarro mentions in his book is Steven Mnuchin, the Trump administration's former Treasury secretary.
Navarro said that Mnuchin and Trump's son-in-law, Jared Kushner, "single-handedly destroyed" Trump's China policy, "with Wall Street and the Chinese Communist Party behind the scenes."
"They're just doing it to make their nest fuller," Navarro said.
Navarro wrote in his book that when Trump asked to label China a currency manipulator, Mnuchin "always refused." Finally, on 5 August 2019, Mnuchin agreed to put this label on the CCP.
However, Navarro said it was too late. He explained in the book that if Mnuchin had agreed to designate China in January 2017, it would "directly and accurately strike one of China's worst abuses" and it would provide U.S. officials " a valuable bargaining chip with China in trade negotiations."
The U.S. Treasury Department revoked the designation on 13 January 2020, two days before Trump and Chinese Vice Premier Liu He signed the Phase 1 trade deal.
China has not fulfilled its commitments in the trade deal. According to the Peterson Institute for International Economics (PIIE), China only buys 57 percent of its promised U.S. exports.
“The reality of that deal was the skinny deal. That’s what it was,” Navarro told NTD. “It was a pale shadow of what it should have done.”
Navarro said Kushner was at fault for the bad trade deal.
“He was double-dealing with Steve Schwarzman on Wall Street and John Thornton, these guys, along with the Chinese Communist Party negotiators, behind the backs of me and [former U.S. Trade Representative Robert] Lighthizer, and even the president to come up with this bad deal,” Navarro said.
Schwarzman is the chief executive officer of U.S.-based investment firm The Blackstone Group, and Thornton is the executive chairman of Canada-based mining company Barrick Gold and a former Goldman Sachs president.
If Trump becomes president again, Navarro suggested the United States should decouple from China.
“I can assure you one of the first things he’s going to do … is lockout tariffs on Communist China,” Navarro said. “We’ll just cut the cord from them because every dollar we pay at Walmart for made-in-China, made-in-Communist-China product, it goes into their military war machine to take out Taiwan.”
“I think it’s really important if we’re going to win back the White House 2024, that we govern over the next four years in a way which gets things done,” Navarro continued. “Time is precious, when you’re in the White House, as we learned, and there were things we left on the table that I wish we hadn’t.”
