According to Reuters news on 10 November 2022, Indonesia's National Transportation Safety Board (KNKT) announced the final findings of the plane's auto-throttle system malfunction, which pilots did not monitor well, which ultimately led to the fatal crash.

The 26-year-old jet's autothrottle system has had as many as 65 repairs since 2013, according to a 202-page final investigation report. Since the pilot can manually control the thrust levers, the autothrottle system does not necessarily need to be enabled when the aircraft is flying.

"In this case, the pilots did not closely monitor the asymmetric thrust issues, including the left engine throttle stick moving back to as low as 34 percent after takeoff, while the right stick stayed at about 92 percent of the original climb settings," Indonesia's National Transportation Safety Board said. 

"There are several indications that the pilot may have checked the aircraft for anomalies such as engine parameters, thrust lever position and roll angle, and pilot overconfidence and bias in confirming flight data may have been a factor in poor monitoring."

Sriwijaya Air did not respond to a request for comment.
