COVID-19 : 94-day-long virus incubation period case suspected in Henan

COVID-19 continues to spread outside Hubei. Recently, Henan officially announced two suspected cases with an ultra-long incubation period, one of which lasted for 94 days. The China Communist Party (CCP)'s official media urgently "repelled rumours." However, Zhong Nanshan, the leader of the expert group of the Health Commission of the CCP, responded that he was not surprised.

On 18 February 2020, Guangdong Province held an epidemic prevention conference. Zhong Nanshan, an academician of the Chinese Academy of Engineering, said, "Not surprisingly, there are always exceptions."

Zhong Nanshan introduced that his previous research on 1099 cases showed that the incubation period was mostly 2-7 days, but one of them reached 24 days and 13 cases exceeded 14 days.

Prior to this, the Xinxian County Epidemic Prevention Office of Xinyang City, Henan Province, reported on 16 February 2020 that there were two confirmed cases in the county. In addition to the long incubation period, they also had unclear sources of infection, difficult diagnosis, and strong infectivity. In one case, a patient by the surname of Wu took 34 days from infection to diagnosis, while another patient by the surname of Ning took 94 days to be diagnosed with covid.

The report stated that Ning  has been living at his father-in-law's house in Baiguoshu Neighborhood Committee of Jinlanshan Street, Xin County since 13 November 2019, and took care of his father-in-law, Zhang, who returned from medical treatment in Wuhan. After the death of his father-in-law on 30 November 2019, Ning continued to stay in his father-in-law's home until 31 January 2020, when he, his wife and son returned back home at the Chendian Township Government Family House opposite the county vocational school. After that, Ning only went out 2 times to buy food, 1 time to buy medicine, and no other outing activities. On 12 February, he developed fever symptoms. On 14 February, he went to the county people's hospital for treatment and then was hospitalized for observation. From the day Ning took care of his father-in-law until the diagnosis was made, the period lasted for 94 days. It is difficult to determine the infection trajectory and the source of the infection.

Xinxian accordingly announced that residents with a history of living, travelling, and contacting in Hubei had their home isolation and observation time extended from 14 days to 21 days.

The news quickly caught the attention of China media and caused heated discussions on social media. On 17 February, the party media quoted "experts" as "dispelling rumours."

China News Agency quoted Feng Zijian, deputy director of the Chinese Communist Party Center for Disease Control and Prevention, as saying that there were 4 cases diagnosed for the first time in Xinxian on 31 January. After Ning returned to the county on 31 January , it was entirely possible that he was infected with the virus while traveling. Then on 12 February, fever symptoms appeared, "consistent with the epidemiological rules of new coronary pneumonia."

Feng Zijian also affirmed, "Although the source of the infection has not been determined, Ning has no direct association with his father-in-law." This statement is clearly different from the official Xinxian report.

At present, people at risk at home and abroad are generally isolated for 14 days but the reality is that many patients have an incubation period of more than 14 days, and some people have no symptoms from beginning to the end. Covid can infect others efficiently during the incubation period. This has caused widespread worries about the effectiveness of CCP's mandatory isolation measures.
